Transformation Program: $1100 paid in full for 2 months 

The complete nutrition care plan + weekly or biweekly virtual consultations

This plan includes: individualized care plan, strategies for dining out, emotional eating, sample menu, travel tips, recipes, grocery store tour, food journal review, weekly or bi-weekly checkups and 12/7 instant message support. Research shows that if clients commit to 8-10 weeks of consultations with a licensed healthcare professional to monitor progress they have a higher success rate for desired outcomes. Expires 16 weeks after initiation of program.

Beginners Nutrition Plan $650

The complete nutrition care plan provides an individualized approach tailored to fit your lifestyle and goals for optimal nutrition and wellness. Clients will receive two virtual nutrition consultations. Expires 16 weeks after initiation of program.

How do I schedule an appointment? 

First you may contact us by email: Mel@FirstChoiceNutritionClinic.com and give your name, preferred contact information, and a description about your needs. We will contact you to schedule your initial appointment and provide you with new client forms.

All completed new client forms must be emailed to me before your scheduled appointment.  Please sign a release of medical records and have your primary care physician's office to email your medical records to my office prior to your scheduled appointment. 


Where do you meet your clients?

I meet all my clients virtually; using video conference or over the phone. In some cases where technology is a barrier, I will provide an in-person session.  

Do you take insurance?

At this time, we do not accept insurance but we do provide you with a superbill at the end of each session. A superbill is an official receipt of services which can be submitted to your insurance company for a reimbursement, depending on your personal insurance plan. Any reimbursement will be sent directly to you from your insurance company. 


 Can I schedule a follow up appointment? 

Initial appointments are required prior to ability to schedule a follow up appointment.

What is the difference between a Registered Dietitian and a Nutritionist?

A Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) is the food and nutrition expert who has accomplished at least a Bachelors degree which coursework included; medical nutrition therapy, nutrition care management, chemistry, organic chemistry, biology, biochemistry, microbiology, food/nutrition sciences, physiology, psychology, sociology and ethics in healthcare. All registered dietitians have completed a 1200 hour supervised practice internship in various facilities (clinical, community, and food service settings) and passed a National Boards Exam administered by the Commission on Dietetic Registration (CDR). Registered Dietitians must complete 75 hours of approved continuing education to maintain their licensure each cycle.

A nutritionist isn't a nationally recognized title and doesn't require a degree or any formal training.  Some states limit nutritionists to provide nutrition counseling;  Registered Dietitians can legally provide nutrition counseling, nutrition diagnosing, treatment of medical conditions and also seek reimbursement from insurance.
