Each patient each day are entitled to receive safe, passionate and respectful care regardless of gender, age, race, religion, language, culture, religion, sexual orientation, socioeconomics, disability, color, race or ethnicity. Care will be provided without any abuse or mistreatment. You have the right to receive care in a manner you can understand and be informed if there are any changes arising.
Patient responsibility
First Choice Nutrition Clinic expects the client to be honest, truthful, respectable and responsible including following instructions, rules and financial commitments. Respect between both parties allows effective and efficient communication to collaborate for quality nutrition care. It is important to communicate openly, share accurate information regarding your personal health and financial commitments, notify us if you need clarification regarding information within the care plan, stay on course with your nutrition care plan, attend appointments on time and cancel/reschedule at least 48 hours prior to your scheduled appointment.
